We will send a health history form
prior to your first visit. Please complete the form and bring it with you
to the appointment. Let us know if you need to take medication before we
work on your teeth. Also, please bring along information on any
medications you currently take so we can record types and dosages. The
more we know, the better we can care for you.
Please let us know if there has
been any change in your medical history between visits so we can update our
records.

We pride ourselves on providing
excellent care to the entire family. We are happy to introduce your
children to modern dentistry and will do our best to make your youngster feel
special and comfortable. At our first visit, we will show your child
around the office. After this meeting we will visually examine your
child's teeth and encourage him or her to come back soon for a cleaning.
We'll show your little one the "special toothbrush" we use.
We are committed to making your
child's dental visits as pleasant as possible. If you have any special

We take x-rays only when it's
absolutely necessary. They help us identify problems and treat them before
they become serious. Besides detecting cavities, x-rays help us uncover
cysts, abscesses, tumors, and even gum disease. They also help us spot
malformed or impacted teeth. If x-rays are taken, we'll go over the
results with you as soon as they're available.
Dental x-rays are quite safe,
thanks to ultra high speed film and digital equipment. If you have any
specific questions on our x-ray equipment, Dr. Hartel will be happy to answer
them.
